ASSESSMENTS
Gaining a deep understanding of our client’s current well-being is often the first step toward improving their situation. We provide medical, social, and home safety assessments.
Our PRI & Screen process incorporates integrity and diligence and is performed by a PRI-certified Registered Nurse. Our PRI Promise to be Prompt, Proficient, Professional, Personal, and Patient-Centered exists in order to ease the tension of an already difficult decision to transition to a Skilled Nursing Facility.
HEALTH CARE COORDINATION
Our health care system is challenging to navigate even under the best of circumstances. We can communicate with providers on our client’s behalf and participate at provider visits with our clients to ensure their medical treatment is understood and optimized. We also offer medication management and monitoring, transportation and therapy coordination, Alzheimer’s and dementia expertise and more.
SENIOR LIVING PLACEMENT & RELOCATION
The question of where an older adult with challenges should reside is highly important and, frequently, not easily answered. Our job is to provide answers and assist families in implementing the optimal situation, whether that includes staying at home with additional care or moving to independent living, assisted living, memory care, or a skilled nursing facility. Together in Caring offers Skilled Nursing Home placement services locally, as well as door-to-door relocation assistance and accompaniment for individuals needing to relocate in the United States who cannot travel alone.
HOME CARE COORDINATION
Aging in place and crisis prevention often require home care. We understand the role of home care and which agencies are best positioned to help in a particular case. We can serve as the home care manager and provide schedule oversight, and monitoring, for the family to ensure the best care is delivered and the burden of coordination is lifted. Home safety assessments can also be performed to offer aging in place recommendations.
ADVOCACY
Sometimes older adults and their caregivers simply cannot be effective advocates for themselves. We are committed to advocating for the best interest in all spheres of an individual’s life, including medical, housing, benefits, legal, and financial.
FAMILY MEDIATION
It is common for well-intentioned family members to disagree about the best interests of their loved ones. We can be a neutral third party to help families navigate difficult decisions together to maximize harmony and minimize stress.
MEDICATION SETUP & MONITORING
Proper medication regimens and adherence is key to aging well in place. Poor compliance is a leading cause of needing to move to a skilled nursing facility and hospitalization. We have nurses on staff who can set up medications, monitor for adherence to prescribed protocols, and communicate with providers and pharmacy to streamline the most effective processes for the client.
REFERRALS TO QUALIFIED MEDICAL, LEGAL AND FINANCIAL PROFESSIONALS
As long-standing aging care providers in our community, we have identified excellent resources for various aging care needs in areas including elder law, home care, health care providers, and more. Since each person is unique, we can help our clients find the best resources for their specific needs and, in the same vein, avoid their becoming involved with people who will not act with their best interest in mind.
SECURING & OPTIMIZING BENEFITS
It is stating the obvious that our “system” of benefits for older adults is complicated to understand and maximize. We know how to find and maximize the benefits to which older adults are entitled. We can help with the logistics of securing benefits and advocating when obstacles are met. This includes long-term care insurance policies, Veteran’s benefits, Medicare, Medicaid, and more.
WELL-PREPARED PROGRAM
Navigating older adults in crisis is more challenging than ever. The COVID-19 pandemic has brought on an extra layer of complexity in an already complex situation. Working together, we gather the information and documentation to enable us to begin our care services within 24 hours should a crisis arise. We will keep in touch regularly and be able to act when a problem or hospitalization arises.
EMERGENCY CONTACT PROGRAM
This program is intended for individuals without local family or support; caregivers with health concerns of their own; and those who want to prepare for the unexpected. We can be the local presence that you can count on during an emergency to advocate for you; execute your aging plan, and be on call as needs arise.
FINANCIAL CARE MANAGEMENT
When budgeting, bill paying and financial matters become a burden for older adults, we can help. The President of Together in Caring has over 20 years of finance and banking experience and is able to assist with bill paying, checkbook balancing, tracking expenses, compiling tax returns, filing and organizing, submitting and tracking claims, and more.
