A Patient Review Instrument (PRI) is an assessment tool developed by the New York State Department of Health, that is required by law to be admitted into a skilled nursing facility. This assessment must be performed by a PRI-certified registered nurse, and is valid for 90 days, as long as the patient’s status has not significantly changed. This assessment is used to determine what type of care and what type of facility is appropriate for the patients’ needs.
Our process incorporates integrity and diligence to the process. Our PRI Promise to be Prompt, Proficient, Professional, Personal, and Patient-Centered exists in order to ease the tension of an already difficult decision to transition to a skilled nursing facility.
To schedule a PRI & Screen, kindly call 585-450-3828, extension 1.

“During a very trying time this organization was a huge help. They were able to provide an unbiased assessment of my mother’s health. The memory care facility that we moved my mom to was one that they recommended. Finally, Together in Caring provided consultant services that allowed us to better understand the ins-and-outs of the Long Term Insurance challenges and potential Medicaid rules. I highly recommend their services.”
“I had the opportunity to leverage the Together in Caring services for my mom. Simply said, the services provided and the people I worked with were all top notch! They took the time to understand our unique situation and helped create a solid support plan that was able to grow and evolve over time. I highly recommend this service!”
